Community Care
CentacareCQ supports wellness, reablement and restorative care approaches in its delivery of home care services. CentacareCQ's Community Care Program:
* Provides comprehensive & coordinated in-home support services, with a focus on wellness, reablement and restorative care for the frail, aged, and people living with a disability.
* Supports our consumers to be more independent at home and in the community, enhancing quality of life to prevent inappropriate admission to long term residential care.
All Services are provided under the Commonwealth Home Support Program, Home Care Packages Level 1-4, and under the National Disability Insurance Scheme. CentacareCQ also provide home maintenance including yard maintenance services and ability to provide community transport services including wheelchair accessible buses.
